JOB SUMMARY: Provides moderately complex (journey-level) case management work. Work involves developing and maintaining long-term contact with clients, client families, and service providers for medical, social, educational, and related service needs. Works under general supervision with moderate latitude for the use of initiative and independent judgment.
DUTIES & ESSENTIAL JOB FUNCTIONS: Under supervision of the Youth Services Coordinator, provides ongoing case management services and rehabilitation services for youth/adults with mental illness. Coordinates and provides services. Demonstrates a positive role model to youth and advocates on their and their families’ behalves. Responsible for coordination, planning and implementation of the treatment plan. Participates in the on-call rotation for crisis resolution for adults and youth.
KNOWLEDGE, SKILLS, AND ABILITIES: Knowledge of youth issues in a rural setting. Ability to deal with schools, juvenile probation and other social service agencies to facilitate services for youth. Ability to communicate with youth and their families to promote compliance with treatment plans. To work independently and remain flexible to solve problems and crisis.
REQUIRED EDUCATION AND EXPERIENCE: Bachelors degree in a human services field from an accredited college or university. Masters degree preferred. Two years direct experience with mentally ill youth in a community setting is preferred. No professional license is required. A valid Texas driver’s license and good driving record is required.
PHYSICAL REQUIREMENTS/WORK ENVIRONMENT: Must be able to pass an agency required physical.
